Application Form

Pick up a copy of the Homeownership Application form (see below) or print a copy from the website
Fill out the form, answering all questions. Incomplete forms will not be processed.
By signing you agree to home visits, employment verification, Credit Check, Criminal Background Check (for all household members over 18 years) and Sex Offender Registry check.
Attach copies of required documents such as driver’s licenses, utility bills, pay stubs, tax forms, etc (a complete list is on the application form)
Mail packet to Habitat address below or drop off sealed packet at OACAC, or Dallas County Health Department.

If your application and credit report meet the Family Selection Criteria, the Selection Committee will verify your rent payment history, work history and set up a home visit.
When the Selection Committee has reviewed all the current applications, they will make their recommendations to the Board of Directors.
When funds and volunteer labor are available, the Selection Committee notifies the selected partner family and the Building Committee schedules the project.
If selected for the next home available, you must complete your assigned “Sweat Equity” hours before receiving your home.
